It's a condition called hypospadias and yes, you are born with it.

An ex of mine had it very severely when he was a babe, and had to have several major corrective operations throughout his life to tackle the condition and the aftermath of surgery due to formation of scar tissue etc.

Was a very difficult thing for him to deal with, especially through his pubescence.
